What are your reasons for collecting personal information?
We ask for contact details to send you appointment reminders, and to send you your Relaxation Track, which you can listen to between sessions. We ask for the name of the GP Practice you are registered with, in the event that we feel it is necessary to contact your healthcare provider. Information will only be shared with other health and social care professionals with your written consent.

How do I know that you will store my information securely?
Your details will be stored on a password-protected personal computer, which will only be accessible to the Data Controller (Tobias Foster).

How long will you hold my information for?
In accordance with National Council for Hypnotherapy (NCH) guidelines, written records will be securely stored for a duration dependent on the age of the client:

- Adult (19 years +) - 8 years after the date of the last interaction with the hypnotherapist.
- 17-18 years old – up to the age of 26.
- 16 and under – up to the age of 25.

What if I would like my data to be destroyed before this date?
Under the May 2018 General Data Protection Regulations (GDPR), you have the right to request the deletion of any of your records at any time.

Can I see or get a copy of the information you hold?
In accordance with GDPR, you have the right to access and obtain a copy of your information within 30 days.

Are our discussions within the hypnotherapy sessions confidential?
Yes, confidentiality is maintained unless there is a need for supervision support or if there is a belief that you may harm yourself or others.

What if I see you outside of a hypnotherapy session?
I (the hypnotherapist) will follow your lead- I will not approach you in a public space or online. If you choose to say hello, in person or online, I would be more than happy to talk with you.
